loot item icon names must match the loot item's 'name' field, but in all lowercase
loot item icons must be png files or they will not load correctly
loot item icons must be 78x78 pixels (they will be resized)
loot item icons will have a 3px border placed on top of them (3px along each side will be occluded)
'default' is a reserved name
